- 1. The subject of the contract is the supply of microtomic knife with handles and autopsy blades with UCK handles in the assortment, estimated quantity and the requirements specified in Annex 3 to SWZ.
2. The removal requires that the offered range meet the requirements for medical devicescontained in the Act of April 7, 2022 on medical devices (i.e., Journal of Laws of 2024, item 1620 as amended) and in the executive acts issued under this Act.
3. Excavations referred to in para. 1 must meet the requirements specified in Annex 3 to SWZ. Failure to meet at least oneFrom the requirements specified in Annex 3 to SWZ, it will reject the offer.
4. The assortment referred to in para. 1, the contractor will be obliged to deliver at his own expense to the UCK Pathomorphology plant, ul. Smoluchowskiego 17.
5. The medical devices referred to in para. 1 will beimplemented on the basis of partial orders. The maximum delivery date is 5 working days from the date of the ordering order by the Ordering Party.
6. Amount allowing partial offers. A partial offer is understood as individual parts set in Annex 3 to SWZ. Performermay submit an offer for one, two or three parts.
