- The order has been divided into 7 packages.
1. The request to confirm the compliance of the offered supplies requests the submission of the evidence listed below in question:
a) a statement that the offered medicinal products are approved at the territory of the CommonwealthPolish and have current registration certificates, in accordance with the provisions of the Act of 06.09.2001 - Pharmaceutical Law and will be made available at each request of the Ordering Party after signing the contract - the content of the statement contained in the offer form constituting Annex 1 to SWZ.healing.
b) a statement that the offered assortment has been admitted and placed on the market, in accordance with the provisions of the Act of 7 April 2022 on medical devices and has current declarations of compliance and certificates of the notified unit - and they will be made available at every requestThe Ordering Party after signing the contract - the content of the statement contained in the offer form, constituting Annex 1 to SWZ. -Dotych medical devices.
c) A statement that the offered medicinal products have the required characteristics cards and will be submitted to each call of the Ordering Party aftersigning the contract - the content of the statement contained in the offer form constituting Annex 1 to SWZ.
2. The removal does not provide for supplementation of the means of evidence in question.
