- 1. The subject of the order is delivered to vascular ports. A detailed description of the subject of the contract can be found in Annex 1a to SWZ.
2. The object and conditions for the implementation of this contract should be in accordance with the Act of April 7, 2022 on medical devices and the Parliament Regulation European (EU) (EU) 2017/745 of April 5, 2017 on medical devices or the Regulation of the European Parliament and of the Council (EU) 2017/746 of April 5, 2017 on medical devices for in vitro diagnostics (if concerned) and with other applicable right regulations In this regard, if the offered item is a medical device.
214 para. 1 point 8 of the Public Procurement Law.
4. The Contractor is obliged to conduct training for medical staff in the scope of the subject of the contract, confirmed by a certificate in the field of service and care of vascular ports, on dates agreed with the Employer. The training includes about 10 people.
5. The exploratory requires a deposit of vascular ports in the amount of 15 pieces.
