- Quotation are invited for Four (4) Years Supply Agreement for IV Administration Safe Set Description SUPP, IV ADMINSTRATION SAFE SET WITH 3 Y SITES β’Needles free system β’3 Y sites, distal one with back check valve β’Depth filter: airtight filter membrane acts as a barrier, protecting against air infusion while retaining particulate matter 15 ΞΌm and below. β’Safe Priming - ensures a closed system β’Luer-Lock fitting for safe connection β’Clear PVC tubing, DEHP-free β’Bacteria-tight air vent, with snap cap β’Good to control flow rate β’Length 220 cm β’Sterile Nos means one single set . Sample required for evaluation, without it, offer shall not be considered. Minimum 2/3rd of total shelf life (wherever applicable) upon deliveries at KOC Hospital stores. Each offer must be accompanied by an Initial Bank Guarantee in Arabic language equal to KD 500 in favor of the Kuwait Oil Company (K.S.C.) and valid for 90 days validity period of the tender, issued in the form of letter of guarantee by any approved local bank. The Tendered shall duty fill in and complete Attachment B to the Form of Tender. The successful Tenderer shall submit a Performance Letter of Guarantee in KWD equivalent to the value of 10% of the contract value. Tender fee amount shall be KD 25 (non-refundable).
