4. Duly Notarized Statement of all on-Going Government and Private Contracts
5. Duly Notarized Statements of:
a. At least three (3) Completed Similar Contracts (Government and Private) for the last five (5) years.
b. Single Largest Completed Contract (SLCC) at least 50% of the total ABC.

7. Project Requirements:
a. Organizational Chart for the Contract to be Bid.
b. List of Contractor's Personnel (Civil Engineer, Skilled laborers [ to include Mason,
Welder, and Carpenter.], and Manual laborers) to be assigned to the contract to be
